Camper Medical Form
The Camper Medical Form must be completed online. The information that you enter will go directly into your camper’s record, which insures 100% accuracy. The Health Center Staff will see exactly what you write.
Note: You will be prompted to enter immunization details. If you don’t have access to this information when filling out the form, there is a check box at the bottom of the page that says, “This person has had no immunizations.” By checking that box, you will be allowed to continue to the next page. Please be sure your physician includes immunization information if it’s not entered on the medical form.
To access the online Medical Form:
Click here to visit the My Campminder Login box. Once there, enter your email address and password. If you have forgotten your password, click the “Forgot Your Password” link to have it emailed to you. After you log in, you will see a link to the Medical Form noted by a red cross.
Certain pages of this form must be printed and returned to camp before May 15th:
- A page for your signature, verifying that the information you entered is correct, and granting certain permissions with regard to medical treatment rendered at camp.
- One or more pages for health professionals, to be filled in, signed, and mailed by them directly to the camp.
- If your child’s health care provider uses their own health exam form, please attach it to the Camp Wayne Medical Form. It is imperative that a Camp Wayne medical form be on file for each camper by the start of camp. No child will be admitted to camp without a properly completed Camp Wayne medical form with a parent’s original signature.
In the best interest of your child, the form should reflect a medical examination performed after March 1st. Please let us know, if you haven’t already, if your child has a medical exam scheduled for after the deadline and when we can expect the Medical Form.
Please send a COPY ONLY of the front and back of your camper’s health insurance card.
